Nearly identical to D-412, this had two booster reels; received Quick Water modifications in 1985; suffered blown engine during November 1992 and replaced with engine from EF-197; converted to a squad in December 1993 by removing pump components and booster tank (more thoroughly than D-395), replumbing deck gun to accept feed from side intakes, and hose body modifications to hold squad equipment, in addition to repainting and affixing white reflective stripe
